Students that plan on participating in high school sports must have the following paper on file with Ms. Sandy Koepke before they will be allwod to partcipate:

• A current sports physical signed and dated by a physician
• Proof of current insurance (a copy must be on file in Ms. Koepke's office)
• Athletic Liability form signed by the participant and their parent/guardian
• CHSAA (Colorado High School Activities Association) form signed by the participant and their parent/guardian
